Uptown Chicago Skilled Nursing & Rehab Center

Our Uptown location provides specialized nursing and  behavioral health programs and support. Using evidence-based and informed practices to support each of our clients' pathways to wellness.

Clients can dine in the center’s dining room for three daily meals. Individual rooms are also equipped with in-room dining services. The Center has a small, private park to enjoy. In addition to medical services provided, clients also receive individual and group therapeutic services as well as recreational activity and a chance to connect with peers.

Individual treatment plans are developed to provide an array of necessary medical supports.
